As the Founding Member General, I have been organizing the Annual
Review Course on Gynecologic Oncology and Pathology since 1990. It is
my greatest joy to be able to hold the 16th course together with all
of you in 2007 as gThe International Symposium on Radical
Hysterectomy Dedicated to Hidekazu Okabayashig
Ê
In 1921, Hidekazu Okabayashi, who was a student of Shouhei Takayama,
Professor and Chairman at Kyoto University, published a paper
entitled gRadical Abdominal Hysterectomy for Cancer of the Cervix
Uteri ? Modification of the Takayama Operation g Surgery, Gynecology
and Obstetrics (1921, p335-341). His operation was a new procedure
that was more radical than the Wertheim method (1911). Since then,
the Okabayashi method has become a standard procedure and has been
contributing to save the lives of numerous patients in Japan with
invasive cervical cancer. However, the Okabayashi method is not as
popular as the Wertheim method in the Western countries. In Japan,
the Okabayashi method has been modified elegantly and now the surgery
is safe and could promise the preservation of nerve function to
achieve better quality of life after radical hysterectomy.

As successors of the Okabayashi and his Radical Hysterectomy, we, the
Department of Gynecology and Obstetrics, Faculty of Medicine, Kyoto
University would like to dedicate this meeting to Professor Hidekazu
Okabayashi, on the 100th anniversary of his graduation by the Faculty
of Medicine at Kyoto University in 1908. As well as dedicating this
meeting to Professor Okabayashi, we would like to discuss the status
of the Radical Hysterectomy which in the 21st Century is changing
drastically: radical vaginal trachelectomy for the preservation of
fertility by Daniel Dargent, laparoscopic radical hysterectomy,
robotic radical hysterectomy, nerve sparing techniques, and so
on.

In this meeting, we can also learn about the basic concepts of
radical hysterectomy from the original classic films in the 19th to
20th century, and modern techniques and concepts of radical
hysterectomy through lectures by distinguished invited speakers from
all over the world.

Dr. Querleu and Dr. Morrow suggested that this symposium may be an
excellent venue to convene a panel of experts to develop a consensus
classification on radical hysterectomy. The only widely used
classification, Types I, II and III of Rutledge and Piver, are quite
unsatisfactory for current practice. Dr. Querleu suggests holding an
expert conference on the classification of radical hysterectomy. This
could propose a new, simple, versatile and surgically relevant
classification with eponyms such as Wertheim, Schauta, or Okabayashi
for terms gproximalh and gdistalh, gwithh or gwithouth nerve
sparing dissection, and completed by glaparoscopich, gabdominalh
or gvaginalh or glaparoscopic-vaginalh.

Moreover, anatomical terminology on radical hysterectomy is
confusing. Through an expert conference on pelvic anatomy, it would
be a great pleasure if we could develop a consensus anatomical
terminology on radical hysterectomy.Ê

The goal for the surgeons to achieve is not different at any period
of time: gA perfect surgery for every patienth. To achieve this
goal, the most important thing is to know the precise anatomy
necessary for the surgery with appropriate surgical skills and a
strong mind to accomplish the perfect surgery. I am convinced that
these four days of the meeting will be a great learning opportunity
for all of us.Ê I hope that many of you will join us and share with
us the joy of learning and achievement on Radical Hysterectomy.

you will surely find the long-cherished tradition still remaining
intact, is waiting for you.
